Over the last 2 weeks, how often have you been bothered by any of the following problems?

Little interest or pleasure in doing things
Feeling down, depressed, or hopeless
Trouble falling or staying asleep, or sleeping too much
Feeling tired or having little energy
Poor appetite or overeating
Feeling bad about yourself—or that you are a failure or have let yourself or your family down
Trouble concentrating on things, such as reading the newspaper or watching television
Moving or speaking so slowly that other people could have noticed? Or the opposite—being so fidgety or restless that you have been moving around a lot more than usual
Thoughts that you would be better off dead or of hurting yourself in some way
If you checked off any problems, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people? not part of the score

What this check measures

The PHQ-9 asks about common signs of low mood over the past two weeks: losing interest in things, sleep and energy changes, trouble concentrating, and feeling down or hopeless. Your answers add up to a single score from 0 to 27.

How the scoring works

Each answer is worth 0 to 3 points. The higher the total, the more low-mood symptoms you are reporting. These bands are a common way to read the result:

ScoreWhat it suggests
0–4Minimal symptoms
5–9Mild symptoms
10–14Moderate symptoms
15–19Moderately severe symptoms
20–27Severe symptoms

What it cannot tell you

A score cannot diagnose depression or any other condition, and it cannot see the context around your life. It is a prompt for reflection and, if the number is worrying you, a good reason to talk with your doctor or a counsellor.

This is a free depression test based on the PHQ-9, the most widely used depression screening questionnaire. It has 9 questions plus one about daily life, takes about three minutes, and is scored the moment you finish, privately, in your browser. It is a screening tool, not a diagnosis.

Questions people ask

Is this a depression diagnosis?

No. The PHQ-9 is a screening tool that measures low-mood symptoms; it does not diagnose. A high score is a good reason to talk with a professional, not a conclusion on its own.

How accurate is the PHQ-9?

It is the most studied depression screening questionnaire in the world, used across primary care. It is good at flagging when symptoms deserve attention; it is a starting point, not the whole picture.

Who sees my answers?

No one. The check is scored entirely in your browser, and nothing you answer is sent to us or stored anywhere. That also means no one monitors your answers; if you need support, please reach out directly.

What should I do with my score?

Save the page first, because nothing is stored here. If your score sits in the moderate range or above, or the number is worrying you, bring it to your doctor or a counsellor; a free consultation is an easy place to start.

This is a free screening tool, not a diagnosis. It can help you notice patterns and decide whether to reach out. Your answers stay on your device; nothing is sent or saved.
